GROUNDED CAMP PARENTAL PARTICIPATION WAIVER & RELEASE FORM

  

As the parent or legal guardian of the named participant (the “camper”), I acknowledge and agree to the following terms and conditions in consideration of the camper’s participation in Grounded Development Ltd. Volleyball camp (the “camp”) operated by Linden Bontkes and Mathias Elser.

 

  1. Assumption of Risk & Liability Release

    I understand that participation in volleyball training and related activities involves inherent risks including but not limited to falls, collisions, contact with other participants, equipment-related injuries, and other accidents that may result in injury. I voluntarily assume all risks associated with participation. I release and hold harmless Grounded Development Ltd. camp organizers, coaches, staff, facility owners, and affiliates from liability, except in the case of gross negligence or willful misconduct.


  1. Medical Consent & Emergency Treatment Authorization

    I certify that my child is physically able to participate. I authorize staff to administer first aid, seek emergency treatment, and transport my child if necessary. I am financially responsible for medical expenses.


  1. Media Release Permission

    I grant permission for photos/videos of my child to be used for website, social media, and promotional materials without compensation. I grant Grounded Camp full rights to use images on the official social media accounts. I acknowledge that is consent is a condition of participation.


  1. Insurance Disclaimer

    I understand that the Grounded Development Ltd camp is covered under a general liability policy through the camp’s insurance provider, which excludes:

-Injury caused by one participant to another participant

-Injuries to performers/entertainers

-Any use of alcohol or drugs

-Illegal activities of any kind

-Use of bleachers or stages

-I understand I am encouraged to carry personal health and accident insurance.

 

  1. Refund & Cancellation Policy

    If the camper is unable to attend, full refund will be given if written notice is provided to groundedcamp@gmail.com 14 days before the camp start date. If after the fourteen day cut off the camper cannot attend due to medical reasons a 75% refund will be permitted if there is a signed doctor’s note provided. There is no refund for missed days.


  1. Code of Conduct

    I acknowledge that the camp will provide adequate supervision, as required by Pacific Academy School facility rental agreements. However, participants must show respect, sportsmanship, and follow facility rules. Failure may result in dismissal. I acknowledge I am responsible for my child’s conduct during the camp.


  1. Coach Availability

    Some coaches may become unavailable if they are called up to their respective national teams during the summer. Regardless, we will maintain our standard 8:1 camper-to-coach ratio and ensure a high-quality training experience.
